Sacramental Documents

If you were Baptized, received First Holy Communion or Confirmation, or were Married at Our Lady of Peace & Safe Journey, then your information was logged into our Sacramental Registers.  We are able to produce a copy of your Sacramental information in the form of a certificate for verification, or simply to have it in your personal documents.  In order for a Sacramental Record to be made into a Certificate, a Sacramental Certificate Request Form is available through our Pastoral Office.

 If you would like to request a certificate, please keep several considerations in mind:
  • The full name (of the applicant) at the time of the Sacrament

  • An accurate date of the Sacrament (e.g. September 1976)

  • The names of the Parents and Sponsors

  • $5 donation

  • Please communicate with our office for clarifications, corrections, etc.
*Be advised that the names reported in the Sacramental Registers are those that were recorded at the time the Sacrament was administered.  In some cases, certain conditions require that the information be updated, or taken note of.  The Pastoral Office will work with you to have these cases clarified.
